Proof of Enrollment Request

If you need Proof of Enrollment, please email Rachel Hotard @ hotard.rachel@stcharlescatholic.org.  Be sure to include your student's name and date of birth.  Mrs. Hotard will send home the completed form with your student.  Typically a Proof of Enrollment is needed to obtain a driver's license or leaner's permit.
