Required Forms for Athletes

Any student wishing to participate in athletics at SCC must complete/submit the following items:

• LHSAA Required Consent Forms  - The LHSAA Required Forms which include the Athletic Participation/Parental Permission Form and the Substance Abuse/Misuse Contract & Consent Form must be completed and signed by the student and a parent/guardian.  It is only necessary to complete these forms once during the student's high school career.
• Medical History Form (Athlete's Physical)  - This form must be completed yearly.
• Birth Certificate - A copy of the student's birth certificate must be submitted during the high school career.
